To make changes to Canada benefits, employees are required to complete and submit the Canada Benefits Change Form, this form can be found under the “Documents” section to the right. The form must be completed and submitted to askpnc@wilburellis.com within 31 days from the effective change date. If you do not inform us of your change within 31 days, we’ll consider the change request to be late. As such, you will have to complete an Evidence of Insurability and be approved for coverage by Canada Life before being allowing the change. If approved, coverage will be effective on the date of approval.
If you have questions, please contact askpnc@wilburellis.com for additional information.
